Please note, this form only needs to be completed if a Bidvest employee has serviced his or her vehicle at a McCarthy Dealership in the months of November and December 2010. Please ensure that the Bidvest employees personal details' are filled in correctly as they will receive confirmation of their complimentary club membership.
